Yes, I understand and agree to comply with all the requirements of membership in the BYU Cougar Band and accept this tuition assistance award for the upcoming season.

No, I am not able to accept membership and this tuition assistance award at the present time because:

Reason for declining

(Optional) While a member of the marching band, scholarship awards can increase by $400 ($200 fall/$200 winter) if you choose to participate in an auditioned concert band (Wind Symphony or Symphonic Band). Auditions for these ensembles occur during the first week of the fall semester. The expectation is participation in the auditioned ensembles for BOTH fall and winter semesters.

I would like to audition for the Wind Symphony or Symphonic Band and receive an increased scholarship amount upon concert band acceptance.
